GET YOUR CARD

Get a Library Card from the comfort of home! While the Library is closed, we’ll issue new Library Cards through our online application below. Simply complete the form and wait for an email to arrive within 24 hours. Your new card will allow you to enjoy all of our free Online Resources!

Proof of Residence

Cards will be issued to those living within the Library’s service area. Proof of residency will be required to extend your card expiration date once the Library reopens.

WHAT DO I NEED TO GET A LIBRARY CARD?

These rules have been suspended while we are closed.

Library cards are available to qualified applicants. Applicants under the age of 18 require the approval of a parent or legal guardian present at the Library with the minor. A photo ID of the adult applicant or the parent/guardian of an applicant under the age of 18 is needed. ID’s with the correct street address must be presented at the time of application.

To change your personal information (change of address or emails) please do one of the following:

Once we reopen: If your ID is not current, please bring your old ID and documents that show your current address (utility bill, property tax statement, rental agreement or home purchase agreement).

WHAT CAN I DO WITH A LIBRARY CARD?

Your Hussey-Mayfield Memorial Public Library card gives you access to our collection of books as well as movies, music CDs, and videos. You can also access audiobooks, computers with internet, eBooks and online content.

HOW OLD DO I HAVE TO BE TO GET A CARD?

Everyone can get a Library card!

  • Date Format: MM slash DD slash YYYY
  • If you are a minor or juvenile, a parent or guardian's signature will be required before the Library card can be picked up and used. Both cardholder, and parent or guardian will need to be present at pick-up.
    Parents may choose a Limited Access Library Card for their minor children. These card holders will not be able to check out R-rated audiovisual materials. Would you like a Limited Access Library Card?
