Proof of Residence

Cards can be issued to those living within the Library’s service area. Proof of residency, such as an active driver’s license, is required.

WHAT DO I NEED TO GET A LIBRARY CARD?

Library cards are available to qualified applicants. Applicants under the age of 18 require the approval of a parent or legal guardian present at the Library with the minor. A photo ID of the adult applicant or the parent/guardian of an applicant under the age of 18 is needed. ID’s with the correct street address must be presented at the time of application.

WHAT IF MY ID HAS AN OLD ADDRESS?

If your ID is not current, please bring your old ID and documents that show your current address (utility bill, property tax statement, rental agreement or home purchase agreement).

WHAT CAN I DO WITH A LIBRARY CARD?

Your Hussey-Mayfield Memorial Public Library card gives you access to our collection of books as well as movies, music CDs, and videos. You can also access audiobooks, computers with internet, eBooks and online content.

HOW OLD DO I HAVE TO BE TO GET A CARD?

Everyone can get a Library card!

  • If you are a minor or juvenile, a parent or guardian's signature will be required before the Library card can be picked up and used. Both cardholder, and parent or guardian will need to be present at pick-up.
