Have you moved recently? You can still access your Hussey-Mayfield Memorial Public Library card. Fill out the form below and bring your ID and proof of current residence. To prove your residency, bring official documents with your address listed (utility bills, mortgage statement, etc.).

WHAT DO I NEED TO GET A LIBRARY CARD?

Library cards are available to qualified applicants. Applicants under the age of 18 require the approval of a parent or legal guardian present at the Library with the minor. A photo ID of the adult applicant or the parent/guardian of an applicant under the age of 18 is needed. ID’s with the correct street address must be presented at the time of application.

WHAT IF MY ID HAS AN OLD ADDRESS?

To change your personal information (change of address or emails) please do one of the following:

  • Change your email in the Evergreen Account
  • Stop by the circulation desk
  • Call the library at 317-873-8340
  • Contact a librarian

If your ID is not current, please bring your old ID and documents that show your current address (utility bill, property tax statement, rental agreement or home purchase agreement).

WHAT CAN I DO WITH A LIBRARY CARD?

Your Hussey-Mayfield Memorial Public Library card gives you access to our collection of books as well as movies, music CDs, and videos. You can also access audiobooks, computers with internet, eBooks and online content.

HOW OLD DO I HAVE TO BE TO GET A CARD?

Everyone can get a Library card! However, if you are under the age of 18, you will need a parent or guardian with a valid ID when picking up your card.

APPLY FOR YOUR CARD

  • If you are a minor or juvenile, a parent or guardian's signature will be required before the Library card can be picked up and used. Both cardholder, and parent or guardian will need to be present at pick-up.
    Parents may choose a Limited Access Library Card for their minor children. These card holders will not be able to check out R-rated audiovisual materials. Would you like a Limited Access Library Card?
